How to Refer Patients

Appointments require a referral from a dental or medical provider

  • LSU School of Dentistry : Referral by dentist or physician only: 
    • Appointments in the dental school clinic are made by referral from dentist or physician. Doctors may fax a referral letter to OMS, indicating the patient's name, contact number, and the specific reason for the referral.  The patient will receive a call from the OMS scheduling clerk within 3-4 weeks. Doctors should call 504-889-9893 or fax to 504-941-8480 to refer a patient.
  • LSU Oral and Maxillofacial Surgery Faculty Practice (New Orleans & Baton Rouge Locations): Referral by dentist or physician only
    • Patients are accepted upon referral from their dental or medical provider.  Please call 504-889-9893, fax 504-941-8480 or email at uomfs@lsuhsc.edu to refer a patient or schedule an appointment for our New Orleans office.  Please call 225-763-3939 for our Baton Rouge office.

  • University Medical Center : Self-referral Clinic:
    • To schedule an appointment, call 504-702-5125.  Dentists or Physicians can fax referrals to 504-702-5742. Appointments are required.
  • Our Lady of the Lake  (7777 Hennessy Blvd, Suite 306, Medical plaza 1, Baton Rouge, LA 70808):
    • ​Appointments can be made by referral only.  For referral information call 225-763-3939. 

Location and Hours

  • LSU Oral and Maxillofacial Surgery Faculty Practice:
    • ​The New Orleans Faculty Practice is located on the 2nd floor of the Annex Bldg at 1100 Florida Avenue, New Orleans, LA 70119.  The hours are Monday - Friday, 8:30 AM - 4:00 PM.
    • The Baton Rouge Faculty Practice is located at the Our Lady of the Lake Hospital, 7777 Hennessy Boulevard, Suite 306, Medical plaza 1, Baton Rouge, LA 70808.  The hours are Monday - Friday, 8:00 AM - 4:00 PM.
  • University Medical Center:
    • The Oral and Maxillofacial Surgery clinic is located in the Clinic Building at 2001 Tulane Avenue on the 3rd floor in Pod 3B.  The clinic hours are Monday - Friday, 8:00 AM - 4:00 PM.
